Filling out the patient registration forms online means that you can save time when you arrive at our office. In order to help us serve you best we kindly ask for assistance by clicking and installing your patient portal below. The portal is a program that you can enter all of your information online and submit securely directly to Bariatric Solutions. Please use the username and password that you received when you contacted our office. If you do not have this information, please contact us and we will set one up for you. If you do not feel comfortable downloading the portal, you can print the .pdf versions of the forms below.
